Registration Instructions:
If you cannot make our last registration event on Sat. August 4th from 9am to 12pm at CVS/Pharmacy please print 2 copies of the form above. Fill them out and sign both. Mail the forms, your payment, your child's shirt/short size, and a copy of your child's birth certificate if required to:

This should be postmarked no later than Sat. August 4th. A copy of the children ‘s birth certificate is required with the registration IF your child is playing for the first time or didn’t play in SFYSL last year.
Fees are as Follows:
Registration per child : $85/1st child, $70/2nd child, then $60.00 each additional child thereafter. (No Opt-Out Fee)
Final Call Registration per child (8/04/12 @ 12pm deadline): $85/1st child, $70/2nd child, then $60.00 each additional child thereafter. (No Opt-Out Fee)
A $25.00/child late fee will apply to all registrations received after 12pm.
XCEL SOCCER CLUB
Please submit Xcel Fees and Registration form (bring copy of birth certificate):
• Academy (practice only) $100/Player
• Under 14/15 Teams $155/Player
• Under 19 Teams $185/Player
Fees include Uniform set & League Fees. Tournament fees not included.
